Being from an electrical maintenance background myself, also being well aware of how solid and homogeneous these connections become after correctly applied explosive or hydraulic crimping, I cannot but agree.
One wonders the last time the joint in question had a preventative maintenance visual or other check. Like the infamous $5.00 'D' shackle, probably 40 years ago when it was installed.
Having installed thousands of joints just like the one that failed I'll put this down to piss poor workmanship.
